Rise of PHM
In the mid-2000s, Adam Sabloff co-created the Ritz-Carlton Residences product, an initiative that led to the first population health management (PHM) program of its kind.

Working together with a leading health system and two Fortune 100 technology companies, Adam and his cross-functional team built a revolutionary model for community-based care for the chronically ill.
Pioneering Technology
This unique PHM model gave rise to VirtualHealth, the first comprehensive cloud-based solution for value-based care.

By creating ground-breaking software architecture and leveraging bleeding edge technologies, VirtualHealth overcame the complex challenges of enabling data to flow seamlessly among the various healthcare silos and providing specialized yet collaborative tools to the entire care continuum.
Client Focus
Within two years of commercial deployment, VirtualHealth surpassed the benchmark of 1 million client lives.

The company’s bedrock is an unwavering commitment to supporting its clients and providing not only the best technology but also an unparalleled level of service to ensure that organizational problems are identified and optimal solutions are delivered.

Through his work, Adam recognized that seniors and the chronically ill residing in Midtown’s numerous development could benefit greatly from next generation care delivery and technology that would make the home an organic extension of the healthcare continuum.
